Is a pipe clamp an acceptable fix?

As an emergency measure on the night, yes. As a permanent repair, no - the pipe is still failing under the clamp.

Why do pinholes keep appearing?

Usually water chemistry, high velocity from oversized pressure, or old thin-wall copper. If it is happening repeatedly we look at pressure and talk about repiping the affected run.

Can you repair without opening the ceiling?

We open the smallest access that lets us make a sound joint. Guessing to avoid an opening usually means opening it twice.
